BE SURE TO MAIL THE MANDATORY FOUR COPIES OF THE SIGNED FORM TO THE PROPER DEPARTMENT OFFICER. KEEP A COPY FOR YOUR DETACHMENT RECORDS. THESE FORMS MAY NOT BE e-MAILED.
These on-line forms can be completed on your computer (may need to be opened in Adobe Reader or other PDF reader). After filling out the form, print it then sign it. The advantage is that all copies are legible. To save your completed form on your computer, “print” to a PDF, then “save” it.
